我國現行共有199家急救責任醫院,須每月至食品藥物管理署「災害防救醫療物資調度支援系統(Medical Resource Dispatch and Support System for Emergency Response, MRDSS)」提報動員藥品量能資訊,本研究對105、106年度各急救責任醫院之提報動員藥品許可證字號提報比率進行分析,以間接了解醫院動員藥品之選用情形。於105、106年度,「Atropine 1 mg/1 mL/amp」、「Calcium gluconate 100mg/mL, 10 mL」及「Water for injection 20 mL/amp」等動員藥品,其年度內最頻繁提報之許可證字號,均佔該年度提報情形八成以上,為極高度提報比率,查藥證系統資料庫均屬於許可證數量較少之動員藥品,而「Penicillins or 1st generation of Cephalosporins」、「Salbutamol inhalation oral 5 mg/2.5mL/amp」、「Gentamicin 80mg/2 mL/vial」等動員藥品,其年度內最頻繁提報之許可證字號,提報比率未達二成五,顯示醫院間對此些動員藥品儲備歧異度高且使用習慣不同,綜上所述,建議應將提報比率高且許可證數量偏低之許可證字號持有廠商加強管理,對提報比率偏低之動員藥品,則應加強督導急救責任醫院對藥品品質的管控,以減少因不同許可證字號所產生的差異性,強化動員藥品之掌控程度。
There are 199 first aid responsibility hospitals in Taiwan. All of them need to report to the "Medical Resource Dispatch and Support System for Emergency Response (MRDSS)" of the Taiwan Food and Drug Administration on the amount of mobilizing drug used every month. In order to indirectly understand the choice of mobilizing drugs in first-aid responsibility hospitals, this study analyzed the license reported rate of the mobilizing drugs in hospitals between 2016 and 2017. Among the most frequently reported license number, "Atropine 1 mg/1 mL/amp," "Calcium gluconate 100 mg/mL, 10 mL" and "Water for injection 20 mL/amp" account for more than 80% of all reporting submissions. However, according to the database of the drug certification system, the amounts of licenses for these drugs were considered relatively low. On the other hand, among the most frequently reported license number, "Penicillins or 1st generation of Cephalosporins," "Salbutamol inhalation oral 5 mg/2.5 mL/ amp" and "Gentamicin 80 mg/2 mL/vial" account for less than 25% of all submissions. This phenomenon indicated high degree of differences on mobilizing drug stocks among hospitals. In view of the above, it is recommended that the competent authority should strengthen the management of license holders with a high rate of submission and a low license numbers. For mobilizing drugs with a low reported rate, it is necessary to strengthen the supervision on controlling drug quality in first aid hospitals to reduce the discrepancies caused by different licenses, and to strengthen the control of mobilizing drug. |
