A 62-year-old man presented to the clinic for fullness sensation of left hip and left thigh with scrotum swelling for 3 weeks. Axial T1 weighted MR image of pelvis showed low-signal-intensity enhancing soft tissue involving bilateral pelvic side wall with more prominent on left side (Panel A, arrow heads). Elevated IgG (1778 mg/dl; normal range:750-1500) and IgG4 (158 mg/dL; normal range <86) were noted. CT-guiding biopsy of this soft tissue involving left pelvic side revealed fibrous tissue with dense lymphoplasmacytic infiltrate (H&E), x100 (Panel B, arrow). The IgG4 positive plasma cells count 15 per high power field and a ration of IgG4 to IgG is 30% (IgG4 immunostain), x200 (Panel C, arrow). |
