A 34-year-old woman with systemic lupus erythematosus (SLE) and antiphospholipid syndrome presented with progressive right blurred vision for one month. She had received treatment with hydroxychloroquine, aspirin and prednisolone for 4 months. Color fundoscopy of right eye showed cotton wool spots (B, arrow) and vascular sheathing (B), which are compatible with moderate retinal vasculitis with ischemic change. Fluorescein angiography showed areas of capillary non-perfusion (A). Optical coherence tomography showed subretinal fluid collection (C). These findings are compatible with vaso-occlusive retinopathy. One month later, she presented with mood disorder and anxiety. Brain MRI was negative but brain perfusion SPECT showed hypoperfusion over right posterior parietal and left parietotemporal region. Neuropsychiatry SLE was diagnosed. She received cyclophosphamide pulse therapy followed with azathioprine. Subretinal fluid decreased after 5 months of treatment (D). Follow up fluorescein angiography 10 months later showed neovascularization with collateral vessel formation, no more capillary leakage. |
