Takayasu's Arteritis Associated with Reno-vascular Hypertension: A Case Report

Takayasu's arteritis是罕見的病因不明性血管炎，它主要發生在生育期年齡女性，而且會侵犯包括主動脈及其分枝的大血管，腎動脈狹窄造成續發性的高血壓是這個疾病的最主要併發症之一，也常造成許多後遺症與死亡。本文描述一個26歲年輕懷孕的婦女罹患此病後，終止了懷孕，並接受血管造形術來治療左鎖骨下動脈及左腎動脈狹窄，這個病人的血管病變是先由頸部的彩色都卜勒音波來篩檢，再經由血管攝影證實，早期診斷與積極治療對改善預後非常重要。

Takayasu's arteritis (TA) is a rare idiopathic vasculitis that primarily affects large vessels of aorta and its main branches. TA occurs predominantly in young women during their reproductive years. Hypertension, often secondary to renal artery stenosis, is the most common major complication in TA. Untreated renal hypertension contributes to the high morbidity and mortality of the disease. We present herein a 26-year-old young female with TA complicated with reno-vascular hypertension, who terminated the pregnancy and received percutaneous transluminal angioplasty in left subclavian artery and left renal artery. The vascular lesions of the patient were initially detected by color Doppler sonography of the neck, and subsequently confirmed by aortogram and renal angiography. Early recognition followed by an aggressive management of the stenotic vessels is critical for improving the prognosis.
