G群鏈球菌性膿性肌炎

Group G Streptococcal Pyomyositis

所謂膿性肌炎，是指骨骼肌的化膿性感染。最近的文獻報告顯示，在非熱帶國家，罹患膿性肌炎的病例持續在增加中，特別是那些具有容易罹患膿性肌炎因素的病人。在罹患膿性肌炎的病例中，有各種不同的致病菌被報告出來，其中，以金黃色葡萄球菌最多。我們報告一例十七歲的女學生，她之前並沒有創傷的病史，也不具有文獻報告過的、容易罹患膿性肌炎的因素。但是，她卻罹患了膿性肌炎。這個由G群鏈球菌所導致的膿性肌炎，是相當罕見的病例。我們借助核磁共振造影和超音波導引下的細針抽取術加以診斷。在抽出病灶液體後，隨之以靜脈注射抗生素加以治療，病人的病情使得到明顯的改善。

Pyomyositis is a pyogenic infection of striated muscle. Recently, there has been an increase in the number of cases in nontropical countries in patients with predisposing conditions. Affected patients present with a variety of pathogens and staphylococcus aureus is the most commonly reported organism. A 17-yearold female student without antecedent trauma or other published predisposing factors, in whom streptococcal pyomyositis developed, is presented. This is an unusual case of group G streptococcal pyomyositis, diagnosed with the aid of magnetic resonance imaging (MRl) and needle aspiration under ultrasound guidance. Following aspiration, the patient's condition responded to intravenous antimicrobial therapy.
