植基於Web介面進線樣板產生器之實作與研究

The Implementation of Web-based Inline Template Generator

表單的本質實際上是一由人所產生的樣板，在樣板中部份為設計者所提供的提示文字，而其他則為取得使用者輸入資料的控制項，如文字輸入方塊、下拉式選單等。對表單管理者而言，若有具彈性且容易使用的表單產生器，可以很容易的產生所需的表單並且可以使用工具分析使用者所輸入的資料。然而目前較常被使用的表單產生器，如微軟Word、Google文件等，所產生的表單均類似問卷調查的格式，對需要樣板為控制項進線表單內容的領域，如醫療資訊系統中病例輸入，傳統的表單並無法提供適當的資料輸入格式。在本研究中，我們開發了一個Web基礎的表單產生器，提供管理者設計進線式的控制項於表單內容中，並可以將使用者所填答的表單轉出為Word格式，供管理者輸出紙本文件。

Basically a form is a template generated by designers, that provides descriptions and controls, such as radio buttons, text boxes and so on, to collect user inputs. For the form designers, a flexible generator would help them to create adequate forms to collect user input and the input can be analyzed with some statistical software later. Commonly used generators, such as Microsoft Word, Google Docs, generate forms that have format similar to some general survey forms. However, in some areas, such as generating medical record in Hospital Information System(HIS), the controls has to be inline to fit some special forms. In our study, we design a web based inline template generator that create forms with the controls embedded inline. The forms filled by the users can be exported to the format of Microsoft Word, that the administrator can then print out the documents for the needs.
