運用Orem護理理論提升精神科急性病房病患身體清潔完成率

精神病患常受症狀影響，導致個人衛生與清潔能力退化較正常人差，所以訓練其維持身體清潔乃護理人員與病患須共同努力的重要課題。精神科疾患在症狀急性期致個人衛生行為缺乏；護理人員因平日工作負荷未能有效督促病患；病房又缺乏對病患身體清潔查核標準等原因，造成病房病患身體清潔完成率低。專案小組除制定標準化身體清潔查核表、建立病患行為修正與身體清潔獎勵辦法，更運用Orem護理理論來提升急性精神科病患執行身體清潔，有效將病房病患身體清潔完成率由57.2%提升為89.5%，除能滿足病患個別性的護理需求，更能提供符合其需要之健康照護。

A patient suffering both mental disease and cancer usually has worse self-clean ability than normal human. It is a tough task for patient and nursing staff to try to train patient who can do simple self- body cleansing. Furthermore, he lose own basic body-clean behavior during serious illness. The ratio of self- body cleansing for mental patient always low due to nurse daily overloading job and no checking standard of patient self-body cleansing. The project team improves body cleansing rate of mental patients upgrade from .% to .% with standard body-clean checking table, building reward regulation of behavior modification and body-cleaning, even raising body-cleaning rate using Orem-oriented theory. The improvements mentioned above can afford not only individual nurse requirement but also more matching of corresponding health care.
