提昇留置導尿管病患對尿袋固定之滿意度

Improvement of satisfaction of patients with long-term indwelling urinary tract catheters

本專案旨在提升留置導尿管病患對尿袋固定的滿意度。對需要長期留置導尿管與尿袋的病患而言，現有固定尿袋的方式除了增加感染與不當拉扯的機會外，還會干擾病患的日常活動，而且外露的導尿管及尿袋更造成病患對自我外觀的不滿，使病患對目前尿袋固定方式的滿意度僅有49.8%，急需改善。經現況分析後，發現：未制定尿袋固定照護標準流程、缺乏相關在職教育、尿袋顯露在外影響美觀、與現有尿袋固定方法忽視病患的活動需求及生理舒適等問題。因此，本病房於2012年11月至2013年5月進行提升滿意度專案。本專案藉由以創新理念所製作的新型尿袋專用袋、制定指導單張與覆核機制、在職教育、以及實際臨床使用等步驟，使病患的滿意度大幅提升至94.8%。期望藉此專案的推廣，以提升以病患為中心的照護目標。

Objective: The purpose of this project is to improve satisfaction of patients who have to indwell urinary tract catheters (UTCs). For the patients with UTC, current indwelling methods could not only increase chances of unexpected infection and inappropriate drag, but also interrupt daily life of patients. In addition, explicit UTC could cause decrement of self-esteem. Methods: After analyzing the results of total 30 questionnaires, no standard nursing process, lacking in-service education, dissatisfied with explicit UTC, and interrupting patients' daily life are found as major factors. Hence, the ward performed this project from November, 2012 to May, 2013. In this project, a new designed UTC bag was used to carry and cover UTC. As well, this project formulated new guideline and review mechanism, as well as in-service education. Results: After finishing the project, those procedures reproved patient satisfaction from 49.8% to 94.8%. Conclusion: New UTC bag had been demonstrated to significantly improve patients' satisfaction. The patients with indwelling UTC were no longer afraid to pe1form social communication. This project had been promoted to other wards, and improved nursing level.
