機構疼痛管理的關鍵：疼痛品質監測指標

Pain Quality Indicator: The Key to Pain Management in Long-term Care Facilities

儘管高盛行率的疼痛和所衍生的後果對機構老人造成極大影響，疼痛管理在機構的重要性不斷被突顯，但老年住民疼痛仍是臨床普遍被忽略的議題。妥善評估與處置疼痛是機構維持照護品質、改善臨床照護困境和考驗機構照顧者照顧專業能力的第一要務。然而，目前國內因沒有具信、效度的長照機構疼痛品質監測指標，致無法提供實務工作人員進行機構內部做疼痛管理品質的監控。據此，本文從疼痛管理的重要性、國內機構疼痛管理品質現況、發展疼痛品質監測指標的理由、機構品質監測、疼痛品質監測指標發展原則述及發展疼痛品質監測指標面臨的挑戰。

In spite of that high prevalence of pain and negative consequences for elders in long-term care facilities and the importance of pain management are addressed, pain management for elders in long-term care facilities is still ignored. However, appropriate pain assessment and management is the first priority to maintain quality of care, improve clinical difficulty and reflect formal caregivers' professional ability. Due to lack of pain quality indicator in long-term care facilities with sound psychometric properties, clinical professionals in long-term care facilities are unable to conduct internal quality control. As such, the aims of this paper is to describe the importance of pain management, current status of pain management quality in institutions in Taiwan, rationale of developing pain quality indicator, quality monitor in institutions, principles of developing pain quality indicator and encounter challenge of developing pain indicator.
