運用歐氏理論協助一位骨科病患自我照顧之護理經驗

Nursing Experience in Applying Orem Theory to Care an Orthopedic Patient

本文是探討一位六十五歲女性骨科疾病患者，因車禍導致右手第二指骨骨折及左腳脛骨骨折對其個人健康之生理、心理上的影響。在個案住院期間90年5月10日至5月20日的照護過程中，藉由歐氏理論進行個案評估，發現個案有：身體活動功能障礙、疼痛、自我照顧能力缺失、高危險感染、調適障礙等五項主要護理問題，針對個案因骨折導致身體活動障礙所產生的身心方面問題訂定有效的護理計劃，加強個案對疾病的認知、態度及自我照顧的技能，於護理期間個案的問題皆已改善，在個案出院返家後持續三次電話追蹤訪問，以確定個案能接受並執行衛教部份，並能自我照顧且獨立的生活。A 65-year-old woman patient with bonlfractures of right hand 2nd finger and left tibia in a traffic accident, was admitted to the hospital from May 10, 2001 to May 20, 2001. We investigated the influence of fracture on her personal health physiology and mental status during her hospitalization. The Orem theory was applied to her and we found five main nursing problems as Follows: Pain, activity defect, self-care ability deficiency, high risk of infection and impaired adjustment.Our goal for the patient was to make her enforce cognition attitude and self-care ability.
