照護一位多發性硬化症患者之護理經驗

The Nursing Experience of Caring a Patient with Multiple Sclerosis

多發性硬化症是一種慢性中樞神經系統疾病，大多侵犯正值生育期之年輕女性為主，突發的漸進式失能狀態，往往使患者身心遭受到強烈的打擊，護理期間筆者經由直接護理病患，運用會談、觀察及傾聽等技巧收集資料；並藉由Gorden十一項健康功能型態作評估，分析出個案四個主要護理問題為：(1)呼吸道清除功能失效；(2)無效性因應能力；(3)身體活動功能障礙；(4)高危險性感染。訂定整體性計畫及措施，再藉由多項護理措施的介入，終使個案的不適症狀獲得緩解，進而改善生活品質。Multiple Sclerosis (MS) is a chronic disease of the central nervous system, which predominantly affects young female during their most productive years. Suffering from suddent attack of neural symptoms causes progressive loss of strong mobility impact. The purpose of this article was to report a nursing experience of caring a patient with multiple sclerosis. The author collected data by interviewing, observing and active listening. By employing the framework of Gordon's functional evaluation, the author concluded four nursing diagnosis: (1)ineffective airway clearance (2) impaired physical mobility (3) ineffective individual coping (4)high risk of infection. Through the nursing process the patient was relieved from the disease discomfort and her quality of life was promoted.
