護理一位鬱血性心臟衰竭病患之經驗

The Nursing Experience of a Patient with Congestive Heart Failure

本個案報告是關於一位鬱血性心臟衰竭病患之護理經驗。護理期間共計16天，筆者利用會談過程分析、臨床評估、觀察及病歷做資料收集，發現病患住院期間主要兩大護理問題為“活動無耐力”及“心衰竭知識缺乏”，故針對問題採取護理措施，協助個案漸進式活動及居家自我照顧知識之衛教，結果個案活動耐力增加，活動量由1MET漸進增加，並於出院前達到3.5~4METs，且對心衰竭知識有顯著提升，隨後順利出院。This case report was the nursing experience of a patient with congestive heart failure. The nursing care period totaled up to sixteen days. The arethor used an analysis of process recording, clinical assessment, observation and chart documents review to collect informations. Two main nursing problems discovered during hospitalization were activity intolerance and poor knowledge of congestive heart failure. Therefore, we focused on the problems of taking nursing steps to help this case with gradual activity and teach him the knowledge of self-care at home. As a result, this case's activity tolerance increased and the activity range from 1MET up to 3.5-4METS. Before discharge, the case improved in his knowledge about congestive heart failure as well.
