本研究的目的在探討雙極性情感病患的服藥歷程,採質性研究紮根理論研究方法,研究者擔任參與觀察者及訪談者,以自擬半結構訪談大綱進行訪談。研究對象以立意取樣,選定某醫學中心精神科門診與急性病房雙極性情感型病患共12位。結果顯示個案當疾病不穩定呈現外援,當病況穩定呈現自制。歷經外援與自制,使得個案對服藥有不同的意義,此意義呈現情意接受度及服藥行為型式。服藥深受人格、病理機轉與大環境等因素影響。希望與信仰是他們服藥歷程中的動力與能源。研究結果有助於臨床醫療人員貼近病患主體的服藥經驗,瞭解其需要與困境,並作為理論發展的基礎。The purpose of this study was to explore the drug taking experiences of bipolar disorder patients by the grounded theory of qualitative method. The researcher assumed the roles of interviewer, using a semi-structured interview guide. Objects of the study were purposive samples. The subjects were selected from outpatients and inpatients with bipolar disorder in a medical center. There were twelve patients in the study. Results: When illness was in its acute stage, taking experiences was considered a stage of external aid. When the patient's illness was stable taking experiences was considered a stage of self-control. Going through the experiences of external aid and self-control, patients' interpretation conveyed to different meaning, each meaning representing in the form of emotional acceptance and behavioral pattern of drug taking. Motivation of drug taking was influenced by factors such as personality characteristicsI=illness course and environment. In the course of time, the patients repeatedly reviewed retrospectively and integrated their drug taking experiences. During the process ,patients were expecting a hope. Beliefs toward treatment also constituted a source of adherence to drug regimen. Conclusion: Medical staff should heighten their knowledge about drug taking experiences of patients. Medical staff could better comprehend the needs and dilemmas of patients and develop better foundation of academic theories. |
