本文旨在探討糖尿病老人在遷移時的壓力與血糖控制之評估及其護理，目的是在針對有遷移壓力的糖尿病老人，進行有效的評估，以及早發現問題，並擬定合宜的護理照護策略。老人在自願或是非自願的情況之下入住護理之家，常因為環境的改變而產生極大的心理壓力，而壓力的反應會在情緒及身體疾病上呈現。糖尿病老人也可能因初遷移到護理之家的壓力而產生負面的情緒，因而影響到血糖的控制。因此，運用新的照護評估方式及照護策略對糖尿病老人遷移於護理之家的血糖管理是必須的。而此評估方式與照護策略的發展，將有助於改善機構對於糖尿病老人的血糖管理。

This article is to explore the assessment of relocation stress and glycemic control in seniors with diabetes and their nursing care in a nursing home. The purpose is to evaluate the effects of relocation stress nursing care and discover patients' problems, and then provide an appropriate nursing care strategy for elderly diabetic patients. The relocation stress occurs as a result of voluntarily or involuntarily relocating an elderly diabetic patient from one environment to another. It can lead to psychological and physiological disturbances. Negative emotions arise with the stress, and thus affect blood glucose levels in those who are moving to nursing homes. Therefore, using new assessment methods and techniques as well as strategies of nursing care for an improved glucose management for seniors with diabetes is considerably essential, which will facilitate greater efficiency of a more integrated diabetes management and service in nursing homes.
