This case study described the nursing care experience of an elder with the fear of falling. The home care was from December 1s, 2011 to March 29th, 2012. Based on comprehensive geriatricassessment (CGA) evaluated the condition of this patient and his caregivers by using skills of physical evaluation, observation, and conversation. The author identified the elder's major health problems including fear of falling, the potential risk of urinary tract infection and increased burden for the caregiver. To improve the home care problem and to decrease caregiver burden, we turned to use the “10-year Plan for Long-term Care in Taiwan” program resources to strengthen the caregiver's caring ability, raise public support and improve home care environmental safety. Increasing the scope of the patient's physical activity and avoiding urinary catheterization decreased the burden of the caregiver. Hence the prognosis was managed with improve the quality of life and increased confidence for both patient and the caregiver. By this nursing experience provide practical example for nursing staffs to reference in the care of these patients. |
