Since 2010, Taiwan Pharmacist Association (TPA) applied and implemented many programs sponsored by Government to promote and protect patient medication safety through pharmacists providing pharmaceutical care in long-term care arena, including home, community, and institutional care. Until 2014, with the growth of the budget and patient number on home pharmaceutical care, the safety issues for pharmacists' home care is getting important. According to the results from a questionnaire survey on pharmacists providing home care conducted by TPA and literature review, this article discusses the types of high risk concerning home care safety and furthermore suggests some resolutions to ensure the safety of pharmacists. Transportation, environments of community and home, Infectious diseases, litigation issues, and the others are the five types of high risk and the alternative strategies to promote home care safety are addressed. |
