「壓力性皮膚損傷」是居家護理所重要品質指標之一，本專案旨在提升居家個案壓力性損傷照護正確率。統計單位於2021年3月2日至31日調查主要照顧者執行壓力性損傷照護正確率為62.6%。調查發現主要因素與主要照顧者照護知能不足、與外籍看護溝通不良、無法即時取得諮詢管道等，經跨專業討論合作，預設正確率目標值達93.9%，設立改善策略，包括制定壓力性損傷作業標準、居家護理師指導完整性及主要照顧者傷口照護查核表、建立LINE群組通訊即時諮詢管道、運用視訊進行傷口換藥正確性監測及指導、連結長照2.0資源運用、每月召開改善成效會議。2021年4月至8月統計主要照顧者執行壓力性損傷照護正確率由62.6%提升至93.9%，期望藉此專案提供同仁參考，提升居家照護品質。

Pressure injury is the important quality indicators about our home care facility. The statistical unit surveyed the main caregivers and the correct rate of pressure injury care 62.6%. The survey results were found the main factors were caring knowledge and ability of caregivers, poor communication between caregivers and carers from different nationalities. The established improvement strategies: Caregiver Wound Care Checklist, establish a Line group communication instant consultation channel, use telecom to monitor and guide et al. the correct rate of primary caregivers performing pressure injury care was increased from 62.6% to 93.9%. Look forward to using this project as a reference to colleagues, and improving the quality of home care.
