Locking plate osteosynthesis is a proven therapy for comminuted proximal humerus fracture (PHF) but locking plates are not covered by the National Health Insurance (NHI) in Taiwan. It is challenging to treat those who cannot afford a locking plate to resolve a comminuted proximal humerus fracture, especially in patients with osteoporosis. This study presents three osteoporotic PHF cases that were successfully treated with conventional plates using the“Plate-on-Plate”method. After surgery, there was bone union without loss of fixation or screw pull-out, as shown in the radiography images for both patients. For all three cases, the union rate was 100%, and there were good functional outcomes. The patients also achieved full range of motion (ROM) for the shoulder after implant removal, as evidenced by an American Shoulder and Elbow Surgeons (ASES) score of 100 and a Constant Murley Score of more than 90 for all three cases. The Plate-on-Plate method prevents the screws from loosening and backing out and prevents loss of fixation in osteoporotic patients. This achieves a satisfactory outcome without a locking plate. |
