A tubo-ovarian abscess (TOA) is a complex adnexal infectious mass most often resulting from pelvic inflammatory disease and commonly presents with adnexal pain, fever, elevated white blood cell (WBC) count, lower abdominal or pelvic pain, and/or abnormal vaginal discharge, though presentation is variable. This report details the case of a 45-year-old woman with G2P1A1 (2 gravida, 1 para, 1 abortus) who presented to the obstetrics and gynecology department with persistent lower abdominal pain and vaginal bleeding. She was diagnosed with a tubo-ovarian abscess and underwent a laparoscopic right salpingo-oophorectomy to remove the right ovary and fallopian tube. On the third day after surgery, the patient suddenly developed slurred speech. This case report shows the occurrence of agrammatic aphasia following surgery. Diffusion weighted imaging (DWI) data confirms an acute ischemic stroke. After medical therapy to restore antegrade blood, the patient’s condition was stable and she was discharged and follow-up care was scheduled in the outpatient department (OPD). |
