The Telemedicine for Drug Addiction during COVID-19 Pandemic Period in One Psychiatric Center: Preliminary Report

探討COVID-19流行期藥癮者的遠距醫療和線上個案管理經驗。本橫斷式研究立意取樣及於收集80位成癮門診個案資料，進行統計。發現81.9%病人滿意遠距醫療，但有9.1%對遠距醫療非常不滿意。63.7%病人擔心遠距醫療門診或遠距心理治療，會讓病人無法反映臨床問題。在線上個案管理方面，91.5%病人對線上個案管理滿意，88.6%認為線上個案管理是有幫忙的。絕大多數病人正面肯定COVID-19流行期遠距醫療和線上個案管理，可改善防疫期間的成癮戒治醫療服務。但遠距醫療和線上個案管理仍有挑戰和困境，如：保護病人隱私、改善遠距醫療介面、提高遠距醫療者品質和優化線上個案管理流程等。

This study explores the experiences of telemedicine and online case management for addiction during the COVID-19 pandemic. In this cross-sectional study, data were collected from 80 outpatients undergoing treatment for addiction by purposive sampling. The collected data were then statistically analyzed. The analysis revealed 81.9% of patients were satisfied with telemedicine, but 9.1% were very dissatisfied with their experience of telemedicine. Concerns were raised by 63.7% of patients that telemedicine consultations or remote psychotherapy might not accurately reflect their clinical issues or their problems might be overlooked by the mental health professionals. In terms of online case management, 91.5% of patients were satisfied with the online case management they used, and 88.6% thought that it was helpful to their treatment for substance use disorder. The majority of patients gave positive feedback about telemedicine and online case management for substance use disorder during COVID-19 pandemic period. However, there were also some potential challenges and predicaments in implementing telemedicine care and online case management, e.g. how to protect patient privacy, how to improve the interface design for telemedicine platforms, how to enhance professional competency of telemedicine among healthcare providers, and how to optimize online case management procedures.
