目的:本研究旨在瞭解台灣與離島兩家衛生福利部醫院護理人員孤寂感、睡眠品質及減效出席的差異。方法:本研究以結構式問卷,普查二醫院病房及重症單位服務至少滿六個月的護理人員;問卷內容包含四個部分:孤寂感、睡眠品質、減效出席及個人基本資料;統計方法包含描述性統計及推論性統計。結果:350位受訪者的孤寂感總分平均值為36.78(SD=9.06),其中180位(51.72%)高於平均值,但醫院間未呈現顯著差異(t=1.03, p=0.30);受訪者睡眠品質總分平均值為7.62(SD=3.60),244位(71.35%)睡眠品質不佳,98位(28.65%)睡眠品質良好,兩家受調查醫院皆有約七成的受訪者其睡眠品質不佳,但未達顯著差異(t=-0.68, p=0.50);減效出席則呈現顯著差異(t=-2.09, p=0.04),本島醫院受訪者的減效出席顯著高於離島醫院受訪者的減效出席。結論:兩家衛生福利部醫院均有超過七成受訪者睡眠品質不佳,醫院應進一步探討護理人員睡眠不佳的原因,提出醫院端可以提供的改善措施,提升護理人員睡眠品質。本島醫院受訪者減效出席顯著高於離島醫院受訪者,本島醫院應積極思考如何降低減效出席。 |

Objectives. This study aims to explore the urban-rural differences in loneliness, sleeping quality, and presenteeism among nursing staff in two Ministry of Health and Welfare hospitals. Methods. This study employed a structured questionnaire to survey nursing staff with a minimum service tenure of six months in hospital wards and intensive care units. The questionnaire, comprising four sections—loneliness, sleeping quality, presenteeism, and personal demographics-was analyzed using descriptive and inferential statistics. Results. Among 350 respondents, the average loneliness score was 36.78, with 51.72% scoring above the mean. However, no statistically significant differences were found between the two hospitals(t=1.03, p=0.30). For sleeping quality, the mean score was 7.62, with 71.35% reporting poor sleeping quality and 28.65% indicating good sleeping quality. Notably, both hospitals had around 70% of respondents reporting poor sleeping quality, and there were no significant differences between them(t=-0.68, p=0.50). A significant difference in presenteeism emerged between two hospital respondents(t=-2.09, p=0.04), with participants from the mainland hospital showing significantly higher rates compared to those from the island hospital. Conclusion. With over 70% of respondents in both hospitals reporting poor sleeping quality, it is essential for hospitals to comprehensively investigate the factors contributing to suboptimal sleep among nursing staff. Identifying and implementing interventions to improve the sleeping quality of nursing personnel should be a management priority. Particularly, participants from the mainland hospital showed significantly higher rates of presenteeism than those from the island hospital. Consequently, the mainland hospital should proactively explore and implement strategies to reduce presenteeism among its staff. |
