Based on the concepts of community health promotion and community health literacy, the local township and a medical authority cooperate to perform an action proposal linking technologic application and medical services in a remote and indigenous area. The program included two parts as below: (1) to promote a health management system with digitalized personal health records including multi-dimensional evaluations and measurements that could trigger an appropriate referral and be shared with visitors’families; (2) to build a community safe net by multi-functional wearable devices with a SOS call-for-help response and a real-time GPS localized show. Within 2 years, there were 900 subjects of 14 communities enrolled and provided satisfactory services that could be an excellent example, named as Fengbin model. |
