Frailty is a precursor to degeneration in elderly individuals, leading to cognitive and physical decline and potentially resulting in disability or paralysis. In our ward, the hospitalization rate for frail elderly patients was 77%, while the integrity of caregivers’care was only 53.9%. This highlighted the need for improvement and served as the motivation for this project. The objective was to enhance the integrity of caregivers’care for frail elderly patients. Several issues were identified in the management of care: caregivers lacked skills and knowledge about frailty care, health education provided by nursing staff was incomplete, there was no routine education or training on frailty care, instructional leaflets were unavailable, and ward activity areas were insufficient. To address these challenges, multiple strategies were implemented, including: providing continuous educational and training programs for caregivers; developing standardized protocols for frailty care; conducting group education sessions; creating health education videos, leaflets, and posters; and establishing a multi-sensory environment within the ward. Following the implementation of these strategies, the integrity of caregivers’care for frail elderly patients improved significantly, increasing from 53.9% to 85.9%. |
