本專案是以提升糖尿病人加入照護方案比率,以及促進糖尿病人照護品質為主要目標,在院方長官指示下,組成跨部會專案小組進行品質改善專案,並運用PDCA(plan計劃、do執行、check確認、action行動)品管手法重新再造空間、流程以及重整組織,重新檢討及改造本院糖尿病人收案動線、空間、組織架構以及人力配置等,運用特性要因分析先分析現況問題攻堅,再運用決策矩陣概念,針對問題擬定並執行,向糖尿病宣戰,目的除了希望提升糖尿病病人加入照護方案比率外,也希望能夠加強糖尿病友的自我健康管理能力,提升照護品質,降低健保醫療財政負擔。專案實施後,本院糖尿病共同照護之糖尿病人加入照護方案比率,由2018年度68.85%,提升至2019年度的78.42%,加入收案人數也由2018年度7,634人,提升到2019年度的9,131人,人數及比率皆顯著成長,糖尿病人對於就醫滿意度亦達95分之水準。 |

The aim of this project is to increase the number of patients of diabetes shared-care network, as well as to improve care quality. Under the supervision of the hospital director, one interdisciplinary task force was teamed up to carry out this project. By using PDCA (Plan, Do, Check, Action) strategy, the diabetes shared-care network at our hospital was reorganized in many aspects. The changes included utilities and space layout, patients flow route, and personnel deployment. Problems were identified and solved by implementing characteristic factor analysis and decision matrix methods. This project also aims to empower patients with self-care abilities to reduce financial burden on the National Health Insurance. The outcome of the project was promising. Inclusion rate of the diabetes shared-care network increased from 68.85% in 2018 to 78.42% in 2019. The amount of patients increased from 7,634 in 2018 to 9,131 in 2019. Both the percentage and number of patients had significant growth. Patients' satisfaction rate of our service was more than 95%. |
