Increasing Focus at an Interdisciplinary Case Conference using the Team Care Focus Matrix

跨領域案例討論會是教學醫院常用的跨領域教育模式，需案例分析工具來協助議題聚焦。許多教學醫院應用Bingham等人提出之健康照護矩陣作為跨領域案例討論的分析模板，但其不容易呈現跨領域成員的任務與貢獻。某醫學中心基於健康照護矩陣精神設計一個案例討論分析的前導工具：團隊照護焦點矩陣，以「團隊醫療照護目標」及「跨領域醫療參與」兩個面向來呈現跨領域成員在照護焦點上的參與程度。此工具可以協助照護團隊討論界定教學會議時需要團隊共同探討的重要議題。約六年的實際運用經驗中，有使用團隊照護焦點矩陣的課程參與者整體滿意度為94.0%，本工具的運用成效良好，希望將此經驗提供給其他教學醫院參考。

An interdisciplinary case conference is a common teaching method for interprofessional education in teaching hospitals. An appropriate case analysis tool increases focus on topics. Many teaching hospitals use the health care matrix that was proposed by Bingham et al. in 2005 as an analysis template for interdisciplinary case discussions but it is difficult to present tasks and the contributions of members from different disciplines. A medical center uses the spirit of the health care matrix to design a leading tool for case analysis: the team care focus matrix.
The team care focus matrix measures the level of participation of different healthcare professionals in team goals in terms of“team care goals”and“interprofessional medical engagement”. Participants cooperate in pre-conference consulting with inter-professional education experts to allow the reporting care team to clarify issues to be discussed in terms of material preparation and performance. After about six years of practical experience, the overall satisfaction rate for course participants who use the team care focus matrix is 94.0%, so this is an effective tool. The authors seek to share the experience with other teaching hospitals in Taiwan.
