一位青少年期腦中風病人的護理經驗

Nursing Care Experience for an Adolescent with Stroke

腦中風有逐漸年輕化的趨勢。腦中風病後會造成長期的失能並影響生活品質。本文旨探討一位腦中風青少年之照護經驗，護理期間為2012年4月27日至2012年5月23日，藉由觀察、會談、傾聽技巧及身體評估，收集主、客觀健康相關資料，並運用Gordon十一項功能性健康模式為評估指引，歸納主要護理問題有身體活動功能障礙、社交互動障礙及無力感等。在照護過程中，給予主動及被動關節運動，以維持肢體活動功能；以同理心與個案建立良好的治療性人際關係，透過主動關懷、陪伴、傾聽的技巧，引發個案復健動機，使其勇敢面對挫折，並學習自我照顧。期望藉由此護理經驗，提供護理同仁照護類似個案之參考，提升照護品質。

The incidence of stroke in young generation is increasing gradually. Stoke may lead to disability and significantly impact the quality of life. This paper described the nursing experience of an adolescent with stroke. The nursing period was from April 1, 2012 to May 23, 2012. The Gordon’s functional health assessment guide was used to assess nursing problem through observation, interview, listening, and physical examination. The nursing problems being identified included mobility impaired physical, social interaction impaired, and powerlessness. During the period of care, the author provided active and passive joint exercise to maintain physical function. Through empathy and establishment of therapeutic relationship, we provided active caring, companionship, and listening to motivate the patient to accept rehabilitation, to face frustration, and to learn self-care. This experience provided nurses with a reference for caring similar patients and improved quality care.
