頸部巨大鰓裂囊腫─病例報告

Giant Branchial Cleft Cyst: Case Report

鰓裂囊腫屬於良性先天性囊腫，常見於孩童期及青少年時期，因囊腫感染腫大而被發現。相對地，在成年人就比較少見。鰓裂囊腫大部分發生在側頸部，與其他先天性頸部腫塊之鑑別診斷包括淋巴管瘤，畸胎瘤，喉囊腫等；此外，在年齡大於 40 歲的病患尚需與囊狀轉移性鱗狀細胞癌（cystic metastic squamous cell carcinoma）作鑑別診斷。外科手術切除為最佳治療方式，復發率低。本文報告一例65歲男性頸部之巨大鰓裂囊腫，以手術切除治療。術後病人復原情形良好無神經學症狀，門診追蹤至今無復發情形。

Branchial cleft cyst is a benign congenital cyst that usually presents within the first two decades of life, manifesting as an enlarging mass due to infection. This congenital anomaly is relatively uncommon in adults. Branchial cleft cyst usually appears on lateral neck and the differential diagnoses include lymphangioma, teratoma, laryngocele and so on. Surgical intervention is the mainstay of treatment, and recurrence is rare. We present a 65-year-old man with a giant branchial cleft cyst receiving excision without complication.
