協助一位門診女性乳癌病人於診斷期及治療期之護理經驗

The Nursing Experience of Assisting an Outpatient with Female Breast Cancer during Diagnostic and Treatment Period

本文描述照護一位正值適婚年齡女性，面臨乳癌診斷確認與接受外科治療之護理經驗。筆者於2011年11月11日至12月25日護理期間，運用觀察、會談與電訪，並應用羅氏適應模式進行護理評估，發現患者於診斷期間有需要乳癌相關訊息與診斷結果確認後所產生之焦慮、傷口復原延遲與身體心像紊亂等身心問題。護理過程中除運用持續性關懷、傾聽及同理心鼓勵個案表達內心感受；並運用專業知識及技能，協助緩解術後所引起之生理不適，改善對身體心像的感受並增進身體功能。經由此次護理經驗，深切地體會到門診護理人員在短暫的門診護理期間，面對個案身心問題時，也能適時介入給予個別性及整體性的護理指導，發揮護理獨特的技能。

This article described the nursing experience of a woman at marriage age facing breast cancer diagnosis confirmation and acceptance of surgical treatment. During the period from November 11, 2011 to December 25, 2011, the author collected data from outpatient department through observations, interviews, and telephone interviews using the Roy Adaptation Model nursing assessment tool. The patient’s health problems included anxiety, postoperative wound recovery delayed, and body image disturbance. Nursing interventions included providing emotional support, providing appropriate information, teaching post operation wound-care procedures, and assisting the patient to accept the breast condition. This nursing experience provides outpatient nurses with a reference of caring similar situation by considering patients’ individual differences and provide holistic nursing care.
