運用安寧理念照護一位急救後恢復自發性心跳之病人與家屬預期性哀傷之護理經驗

A Nursing Experience of Hospices Care for a Patient with Cardiopulmonary Resuscitation and Family

本文描述一位經急救後恢復自發性心跳的年輕病人於加護病房接受安寧善終，運用安寧理念協助家屬面對獨子即將死亡的預期性哀傷之護理經驗。照護期間為2018年1月9日至1月17日，筆者進行主客觀的資料收集，發現主要健康問題為：病人的水腫、潛在危險性皮膚受損與案父的預期性哀傷，在照護過程筆者與案父建立信任關係並及早介入安寧療護，藉由宗教信仰的力量協助案父渡過獨子即將死亡的哀傷，期望重症加護的護理師除了照護治療病人的生理問題，在面對案父預期性哀傷，也能夠主動發現並協助案父渡過面對親人死亡的哀傷情緒，期望藉由此安寧個案照護經驗分享，提供臨床護理師日後照護參考。

This article discussed the experience of hospice care for a young man with cardiopulmonary resuscitation in the ICU. The hospice care also extended to his father who had anticipatory grief related to facing the death of his son. The nursing period was form January 9 to 17, 2018. After evaluation, the nursing problems were determined as: edema, impaired skin integrity, anticipatory grief with father. During the nursing care, the author built trust with his father and used to religion to relieve his father's anticipatory grief related to facing the death of his son. The nursing experience not only included care for the patient but also extended it to his family. By sharing this experience, we hope ICU nurses can appreciate the unique role they can play to enhance the quality of care to the patients and their family members.
