照護一位重症肌無力患者之護理經驗

The Nursing Experience of Caring a Patient with Myasthenia Gravis

本文描述一位56歲重症肌無力男性患者的護理經驗。筆者於2016年2月17日至3月12日護理期間，運用Gordon十一項健康功能評估為工具，透過直接護理、觀察、筆談、身體評估及病歷查閱等方式進行資料收集，確認患者有低效性呼吸型態、營養少於身體所需與無力感等健康問題。透過主動關懷、陪伴、傾聽筆談來建立護病關係，藉由呼吸復建運動可改善呼吸通氣維持氧合改善，提供高熱量及高蛋白飲食來維持身體營養需求，結合跨領域團隊照護幫助患者及家屬能積極面對疾病導致身體心理的不適，及接受良好治療和預防疾病再度復發，提升患者及家屬對疾病的認識及提供日常照護能力，進而改善生活品質。期望此照護經驗能與同儕分享，並作為日後照護此類個案之參考。 This article described the nursing experience of caring a 56-year-old male with myasthenia gravis. During the nursing period from February 17th to March 12th, 2016, Gordon's eleven health function assessment tools were used to collect data through direct care, observation, documentation, physical assessment, and review of medical records. The identified health problem included ineffectiveness breathing patterns, nutrition needs less than the body requires, and powerlessness. Through active care, companionship, and listening, the nursing team establish a good relationship with the patient. Respiratory rehabilitation exercise was provided to improve respiratory ventilation and maintain oxygenation. High calorie and high protein diet were provided to maintain nutrition needs. Multidisciplinary team helped the patient and family face the discomfort of illness provided appropriate treatment and prevented recurrence, improved disease awareness, and provided day-to-day care to improve the quality of life.
