腎病症候群合併痛風－－病例報告

Idiopathic Glomerulonephritis and Gout - A Case Report

腎病症候群很少合併關節炎，文獻只有幾例合併類風溼關節炎及類澱粉沉積的病例報告。我們報告一例55歲婦女首先發生腎病症候群，經給予利尿劑及類固醇後，水腫有顯著改善。但一年後，病人的右膝有明顯的紅腫熱痛，且病人有高尿酸血症。經口服indomethacin及關節內注射triamcinolone後，症狀得到緩解。由於此病人停用秋水仙素及加nzbromazone後症狀又復發，所以我們診斷為腎病症候群合併痛風，並回顧相關文獻。

Nephrotic syndrome had rarely been associated gouty arthritis. We report a 53 year old woman with idiopathic glomerulonephritis who developed acute monoarthritis of right knee after 23 months of diuretic therapy for nephrotic syndrome. The symptoms improved after indomethacin dramatically, gout was diagnosis. We will discuss the case and review of the literatures.
