照護一位大腸直腸癌併惡性腸阻塞護理人員之護理經驗

A Nursing Experience of Caring a Nurse having Colorectal Cancer with Malignant Bowel Obstruction

本文探討一位護理人員罹患大腸直腸癌合併惡性腸阻塞之護理經驗。護理期間自2011年10月04日至11月01日，運用Gordon十一項健康功能型態評估。護理問題有營養少於身體所需、角色扮演不當、抉擇衝突及焦慮等。首先處理營養少於身體所需缺失造成的生理不適再經由持續性的照護陪伴、傾聽過去的成就與目前的害怕，協助角色轉換；尊重其對治療的選擇，陪伴渡過抉擇的衝突；術後提供的飲食相關知識，增加病人對進食的信心。藉此經驗提醒，儘管「病人」的角色是由「護理人員」轉變成，仍須重視其護理問題，落實「以人為中心」之照護理念。

This case report describes the nursing experience caring for a female patient with obstructive colorectal cancer. We collected the data from October 4, 2011 to November 1, 2011. Gordon's functional health pattern was used for evaluation guidance. The nursing issues identified were suboptimal nutrition, ineffective role performance, conflict in decisions making, and anxiety. Nursing care first started by helping the patient improve nutrition, which led to better physiological comfort. Other nursing measures included listening to her past achievement and current fear, the role reversal from nurse to patient, assisting her in her with making decision about treatment options, and providing her full dietary knowledge regarding bowel surgery, which increase her confidence in choosing what eat.
