以團隊模式建立新病患安全復健治療環境

A New Patient Safety Rehabilitation Setting: By Team Work Approach

病患安全是二十一世紀健康照顧最重要的醫療指標，就復健醫療而言，要建立一個病患安全的復健治療環境，是必須以病患為軸心，藉由復健治療團隊的各專業人員，包括：醫師、專業治療人員、護理人員及病患照顧人員，甚至病患本身，都必須積極預防疏失的發生，並不斷從過去疏失中學習。一方面在所有醫療人員間建立“安全文化”，著重加強知識的更新、技術及工具的改善發展、並重視結果的分析與改良，以達到病患安全的目標；另一方面必須不斷更新醫療資訊，隨時給予病患本身及照顧者最正確的安全資訊，來建立“以病患安全為中心”的復健醫療照顧。這正是新世紀醫療照顧理念所強調的多專業性、團隊性的整合照顧模式。本篇文章分別針對醫師層面、護理照顧層面及復健治療層面這三方面，整合過去學者的相關研究結果，期望借助跨專業性的一致化、全人化的標準照顧，來建立新病患安全復健治療環境。

Patient safety' is one of the most important medical aims of health care in the 21 century. For a patient safety rehabilitation setting, the patient is the axis combined with the members of rehabilitation team, including physiatxists, specific therapists, nurses and caregivers. Even the patient himself, it is necessary to aggressively prevention from mistake and continuously learning from accidence. In order to the target of patient safety, all medical members should have concepts of culture of safety, emphasize on renewing their knowledge, develop new techniques and tools and be serious about analyses of results and modification. Besides, we must continuously renew medical concepts and give correct safety information to the patient himself and the caregivers for setting a patient s afety rehabilitation program. It is the multi-expert and group care model that promoted by health care members in the new century. This subject reviewed past study of patient safety from the aspects of physicians, nurse care and rehabilitation, by the unity of inter-expert and standard care of whole people to set a safer rehabilitation environment.
