市售肝素製劑之品質調查

QUALITY INVESTIGATION OF HEPARIN PREPARATIONS

肝素製劑是用於治療血栓症的藥物，其主要作用為抑制血液凝固。目前市售肝素製劑以輸入品佔多數，除在查驗登記時接受檢驗外，進口後很少再抽查檢驗，對於此類製劑品質實有調查的必要。肝素原料或製劑的效價測定法有很多種，世界衛生組織（WHO）於1984年公布的肝素第四國際標準品 （4th International standard, 4th I.S.），其活性為 1780IU/ amp.，就是由二十二個實驗室以數種測定法共同標定而成。但是各國藥典所收載的肝素效價測定法及其標示單位都不一致：美國藥典 XXI 版是測量 Recalcification Time （RT），以 USP unit 為標示單位；而英國藥典1980年版及日本藥局方1986年版都收載以硫酸鹽化牛血為受質，測量 Prothrombin Time（PT）；但英國藥典1983年補篇（Addendum）則捨棄已採用30年的 PT 法，而使用無血小板羊血漿（plateletfree sheep plasma）為受質的 Activated Partial Thromboplastin Time （APTT）法，與歐洲藥典1984年版所採用的方法相似；且英國藥典，日本藥局方及歐洲藥典都使用 IU 為標示單位，但USP unit 與 IU 不相等。各種藥典測定法中，PT 法較為煩瑣；RT 與 APTT 法較常使用，但 RT 所使用的材料及操作步驟較簡便，因此本調查使用美國藥典 XXI 版的 RT 法進行效價測定。

In order to investigate the quality of heparin preparations, 28 preparations were purchased from Taiwan area in July, 1986 till June, 1987. Among them, 25 preparations were imported products and other 3 were domestic products. According to the assay method of Heparin sodium injection in USP XXI, the results showed that all preparations fulfilled the requirement of being not less than 90% and not more than 110% of the stated potency.
