約12％的上皮性卵巢癌是發生在停經前，根據研究報告，分化良好的第Ⅰ期A上皮性卵巢癌，被認為是可以採取保守性治療。然而育齡婦女的上皮性卵巢癌，如僅採用保守性治療，可能會增加病灶的復發並影響生存，但卻有保留懷孕的機會。當然在過程中必需嚴密追蹤，使能早期偵查出復發的病灶。為避免醫療糾紛，必須先詳細與患者及家屬說明得失。

Approximately 12% of patients with epithelial ovarian cancer are premenopausal. Previous studies largely support the conservative treayment of stage IA, grade 1 epithelial ovarian cancer in women desiring future gertility. Conservative management of epithelial ovarian cancer, including clear cell type, may probable increase the risk of recurrence but dose not affect overall survival significantly. However, such management offers patients with early disease the chance to have spontaneous pregnancy. Conservation management might be proposed in young patients who wish to preserve their fertility, but careful follow up will be required to detect tumor recurrence.
