建置高齡友善健康照護環境之實務政策──以國軍高雄總醫院為例

The Practical Policies of Developing Age-friendly Health Care Environment: An example of Kaohsiung Armed Forces General Hospital

臺灣人口結構快速轉變，其高齡化速度比日本還快速，雖然高齡長者對於醫療的需求度較高，然而醫療機構對於高齡友善的服務仍有待改善；國民健康署在2010年開始推動「高齡友善健康照護機構認證」，其導入架構涵蓋：「管理政策」、「溝通與服務」、「照護流程」與「物理環境」等四大標準，提供醫療機構作為導入參考及實施之自我評量； 國軍高雄總醫院為建置一個安全、有愛無礙的友善環境，以「專業安全護高齡、溫馨友善關懷您」為推廣宣言，積極推動「尊重長者在健康照護上自主參與的能力與權利」、「符合長者需求，建置便捷、貼心的就醫環境」、「成立高齡照護專業團隊，給予整合性的完善照護計畫」以及「深耕在地化，提供社區長者健康促進知識及關懷服務」等四大高齡友善政策，並獲得國民健康署「高齡友善健康照護機構」之認證，秉持以高齡長者為中心，提供全人且連續的健康服務，建置增進長者健康、尊嚴的高齡友善健康照護環境。

The population structure is changing rapidly in Taiwan that aging rate is faster than Japan. The elderly adults have relatively greater demand of health care. However, the healthcare institutions for age-friendly services are still to be improved. Since 2010, the Health Promotion Administration has launched the accreditation of 'Age-friendly Healthcare Institutions'. The import frameworks in four major criteria provide healthcare institutions for the introduction and implementation of self-assessment, including 'management policy', 'communication and services', 'care process' and 'physical environment'. In order to set up a friendly environment of safe and barrier-free, the Kaohsiung Armed Forces General Hospital promotes the declaration including 'professional and safety care for the elderly' and 'warmth and friendly care about you'; Four major age-friendly policies including 'respect for the ability and rights of the elderly that participate independently in health care', 'accord with the demands of the elderly and set up the convenient and considerate environment for medical treatment', 'establish the professional healthcare team for the elderly and formulate the integrated care plan' and 'localize deeply and provide health promotion knowledge and care services for the elderly in community'. The hospital has achieved the accreditation of age-friendly healthcare institutions by the Health Promotion Administration. Centered on the elderly and provide holistic and continuous health services for improving the health of the elderly and the dignity of age-friendly healthcare environment.
