新生兒足跟採血導致足部瘀傷改善方案

本案旨在降低新生兒因足跟採血所致足部瘀傷率。經現況調查發現：足跟採血所致足部瘀傷發生率約31.5％，其中在嬰兒床中接受足跟採血瘀傷發生率為27.8％，高於保溫箱中採血瘀傷發生率3.7％，深入調查後發現保溫箱中新生兒足部皮膚溫度及溫度穩定度皆高於在嬰兒床中，推測足部瘀傷與溫度有關，故針對在嬰兒床中接受足跟採血進行改善對策，以降低其足部瘀傷率。進行對策之過程為：一、設立溫暖足部標準作業；二、宣導及安排教育訓練；三、實地查檢執行情況；四、持續監測足部瘀傷發生率。經實行改善方案後，嬰兒床接受足跟採血之足部瘀傷率由27.8％降至8.2％，下降率為70.4％，總瘀傷率由31.5％降至12.1％，有效提昇照護品質及達到專案目的，希望藉由此專案分享以提供同仁臨床上的參考與應用。
