侵入性肺麴菌病－－病例報告

Invasive Pulmonary Aspergillosis - Case Report

台灣位於亞熱帶地區，四面環海其氣候及環境非常適合黴菌之生長，所以在黴菌感染的治療上，更顯的非常重要。對於治療侵入性黴菌感染，在傳統上的抗黴菌藥是以Amphotericin-B爲主，但其治療上卻存在著腎臟毒性大，以及僅有注 射劑型……等等的缺點，所以在治療上便有所限制，但對於侵犯性黴菌感染的治療仍是最有效的藥物之一。而衛生署於92年11月核准Voriconazole上市，對於侵入性麴菌症之治療提供了一個除了Amphotericin-B之外的另一種選擇，並且Voriconazole不僅有注射劑型，且提供病患更便利的口服製劑，但由於臨床使用上的經驗較少，因此藉由此病例之探討，希望能提供臨床上治療之幫助。

Taiwan is located on the subtropical area and has oceans around all sides. The damp climate and the environment extremely suit for the mold to growth. Hence, the treatment for the mold infection reveals count for much. Regarding to the treatment of invasion mold infection, Amphotericin-B is the traditional anti-mold medicine primarily; but the shortcoming is high risk to cause kidney toxicity, as well as only the injection dosage-form and so on. Although the limit to some extent on treatment, it was still one of the most effective medicines for treatment of invasive mold infection. The department of Health approve Voriconazole in November of 2003. It has offered another kind of choice besides the Amphotericin-B to the treatment of invasive fungus disease; and Voriconazole not merely has the injection dosage-form but also offers a convenient oral preparation to patients; however, the experience in the clinical use is less. Therefore according to this case of illness, we hope to provide help in the clinical treatment.
