醫學中心與區域醫院在總額預算制度下經營效率及生產力之變化

Efficiency and Productivity Change of Medical Centers and Regional Hospitals under the Global Budget System

本研究主要探討醫學中心與區域醫院在全民健保實行醫院總額預算制度 後，在經營效率表現上的差異及變化。本文以實施當年（ 91 年） 前一年（ 90 年） 以及後兩年（ 92 、93 年） 資料作分析。除了運用資料包絡分析法（ DEA ） 外，再以Malmqui st 總要素生產力指數法（ MPI ） 測量民國90-93 年期間醫院的 總要素生產力變化情況。由DEA 模型結果得知， 90 與91 年度醫學中心的技術 效率值高於區域醫院， 92 年區域醫院表現優於醫學中心（可能受到SARS 的影 響），93 年兩者表現不分軒鞋， 顯示兩者的表現差距在逐漸縮小當中。而MPI 實證結果顯示， 制度實施後，造成醫學中心與區域醫院總要素生產力退化的 主要原因為技術退步，且醫學中心退化程度較區域醫院嚴重。由上述兩種模 型結果來看， 可能因為制度實施後， 醫學中心要在短期內調整龐大的醫療資 源有其困難度在；而醫學中心較區域醫院微底落實總額制度政策，控制醫療 服務量情況下， 產出表現亦不如區域醫院，因此逐漸被趕土， 總要素生產力 退化程度也較區域醫院嚴重。因此，如果將來政府相關單位未加以追蹤各醫 院適當的醫療服務量， 較大之醫院也沒有控制內部要素投入量的危機意識， 相信不久後必定會面臨比較小之醫院更加窘迫的財務危機c

The purpose of this paper is to measure the efficiency and productivity change of medical centers and regional hospitals under the Global Budget System. By Data Envelopment Approach (DEA) we can know the relative efficiency between these hospitals. We also measure the productivity change during 2001-2004 by Malmquist productivity index (MPI). Empirical results indicate that the efficiency difference between medical centers and regional hospitals had become smaller. Medical centers performed better than regional hospitals in technical efficiency in the first two years (2001 2002) but regional hospitals performed better than medical centers in 2003, almost equal in 2004 under the DEA model. Moreover, the productivity of medical centers and regional hospitals were decreasing under the Global Budget System. Technology was the main factor to cause the decrease in productivity.
