病人安全相關議題之探討

Exploring the Issues on Patient Safety

降低醫療誤失（medical error） 以確保病人安全（patient safety） 已成世界各國 健康照護服務體系的重要議題之一。本文昌在探討病人安全的相關議題，首 先探討病人安全及醫療誤矢，其次探討醫療機構內常見的醫療誤失包括用藥 錯誤、醫療機構內戚染及醫療機構內跌倒，進而提出有效提高醫療機構內病 人安全的方法，包括在醫療機構內建立良好的溝通環境、領導權（leadership） 的 建立、採用實證醫學（evidence-based medicine） 的原理、主動上報醫療誤失及使 用電腦網路，可供醫療機構管理者及衛生政策制定者參考。

How to reduce medical errors to assure patient safety has become an important issue in health care delivery system for many nations. The purpose of this article is to discuss the issues in relation to patient safety. First, this paper defines and explores patient safety and medical errors. Then the most common medical errors in health institutions including medication errors, infections, and falls are introduced in this paper. Finally, this paper proposes ways which can increase patient safety in health institutions including to create an excellent communication environment, to build leaderships, to adopt evidence-based medicine, to report medical errors voluntarily, and to use internet. These suggestions can provide the references of patient safety for administrators in health institutions and policy makers.
