The Resolution to Improve the Accuracy of Fall Assessment in Medical Ward

背景：醫院發生意外事件中，以住院病人跌倒的比率最高，正確發現病人潛在的跌倒高危險因子及時給予護理指導，可降低跌倒的發生機率。目的：提升護理人員跌倒高危險因子評估的知識及技能，降低跌倒發生率及傷害率。解決方案：提升內科病房護理人員跌倒高危險因子評估正確率，包括舉辦教育訓練課程、設計提醒卡、病人下床如廁次數統計表及定期稽核機制。結果：執行跌倒高危險因子評估正確率由改善前73%提升為87%，目標達成率116%；跌倒傷害率由44%降為23%，目標達成率117%。討論：跌倒高危險因子評估正確率提升的確降低跌倒傷害率，在護理照護品質及醫療處置的即時性上都獲得改善，確實提供病人一個安全的、高品質的醫療環境。

The highest ratio of the accidents in hospitals is fall of inpatients. To find out inpatients’ potential high risk factors of fall in advance and give nursing care with prevention measures which could reduce the incident rate of fall. The purpose is improved nursing knowledge and skills of the high risk factors evaluation in order to reduce the rate of fall and injury, further to promote inpatients’ security. There are four ways to improve accuracy rate of the high risk factors evaluation of fall in medical ward nurses, 1. Performance training course; 2. Design reminding card; 3. Calculate the number of times of inpatients go to toilet; 4. Regular auditing mechanism. The accuracy rate of high risk factor evaluation of fall by nurses’ performance from 73% increased to 87%; the target completion rate is 116%. The fall and injury rates of the unit decline to 23% from 44%; the target completion rate is 117%. Improve the accuracy rate of high risk factors evaluation of fall by medical ward nurses which can reduce the fall rate of inpatients. This also promotes nursing care quality and medical treatment. Further provide patient safety, high quality medical care environment.
