醫師對出院準備服務的認知、態度、行為及其相關因素之研究－－以南部某醫學中心為例

A Study of Physicians' Knowledge, Attitudes and Behaviors toward Discharge Planning in a Medical Center

背景：醫師為出院準備服務推動成功的主要關鍵因素，但國內關於醫師對出院準備服務之認知研究卻甚為缺乏。目的：瞭解醫師對出院準備服務的認知、態度、行為及其相關因素之探討。方法：採橫斷式研究設計，針對南部某醫學中心228位醫師採結構式問卷調查。結果：（1）醫師對出院準備服務的認知總平均得分為56.7分，屬於中等程度。（2）醫師對出院準備服務持正向態度。（3）醫師在出院準備服務的行為得分平均為3.5分，介於「尚可」至「經常如此」。（4）是否曾接受過出院準備服務教育課程與態度為行為之預測變項，共可解釋行為24.5%之總變異量。結論：根據研究結果，建議在醫學養成教育中應規劃出院準備服務課程，讓醫師於進入醫院前就瞭解出院準備服務之精神，進而提高其配合度，使出院準備服務可成功推動。

Background: Physician is a significant person to improve discharge planning successfully. However, it has limited study to have a regard for discuss physician’s readiness and the related factors in discharge planning. Purpose: The aims of this study are to investigate the knowledge, attitudes, and behaviors of discharge planning and the related factors among physicians. Method: A cross-sectional design was applied in this study. We recruited 228 physicians from a medical center in south of Taiwan. The data collection was by a structured questionnaire. Results: (1) The average score of physicians’ knowledge about discharge planning was 56.7. (2) Physicians have positive attitudes toward discharge planning. (3) The average score of physicians’ behaviors about discharge planning was 3.5. It means their behaviors were between “sometimes” and “usually”. (4) Logistic regression analysis showed physicians’ attitudes and physicians had received discharge planning educational training were significant predictors of physicians’ behaviors, explaining 24.5% of the variance. Conclusion: According to the results, we suggested to design a discharge planning curriculum into the medicine education program. Physicians could realize the importance of the discharge planning before entering the hospital. It would enhance their implementation of initiating the discharge planning.
