Objectives: Establishment of an internal incident reporting system is critical to secure patient safety in the hospital. In this study we report the effectiveness in the establishment of an on-line patient safety reporting system. Materials and Methods: By using J2EE platform, we followed the Taiwan Patient-Safety Reporting System (TPR) standard operation procedure to establish an in-hospital patient safety reporting system. The system is designed to map the TPR database variables, to convert incidence reports into XML format, and to upload data to the Taiwan Joint Commission on Hospital Accreditation (TJCHA) automatically on a monthly basis. The in-hospital patient safety system aims to prevent incidence from recurrence by collecting and analyzing incidence information in advance. In this study we used a “Patient Safety Reporting System Survey” to compare users’ perception and satisfaction with different reporting channels (on-line system and paper form). Results: The on-line reporting system significantly enhances user satisfaction. The effectiveness is mostly remarkable in management efficiency, confidentiality, multi-media integration, and data analyses. We found the establishment of this on-line patient safety reporting system has successfully improved the willingness to report in our hospital. Conclusion: The establishment of an on-line patient safety reporting system proves to be effective to improve incidence reporting efficiency, confidentiality, and quality. |
