The purpose of this article is to describe the nursing experience of an ESRD patient on peritoneal dialysis .The author utilized Gordon's 11 Functional Health Patterns to collect and analyze information then identify problems based on physical assessment, close observation and intensive interviews with the patient. Major health issues found included: the patient's lack of knowledge, potential risk of infection, and the patient's sense of hopelessness. The author established a positive patient-caregiver relationship through proactive care and intervention to resolve the problems associated with the potential risk of infection and the sense of hopelessness. Professional guidance and knowledge of care planning were also provided for the patient on peritoneal dialysis with the goal of eliminating their concerns, anxiety as well as help the patient take care of themselves in the future. The care experience in this article will hopefully provide other nursing personnel with a reference for future clinical care. |
