The Role and Function of Nursing in Chronic Renal Insufficiency Care

腎組織灌流不全是臨床上常見的護理問題，一般人可能會以為僅是腎臟的一個小問題或是腎功能不全，實際上腎組織灌流不全影響的將是全身性的問題，若未能早期發現，即時監測治療，其對腎功能的毀損亦將成為不可逆的病變。然而何時該為病患立此護理診斷卻一直困擾著護理人員，甚至儘管已經為病患確立此問題，對它的界定仍然感到模糊。因而引發本文的動機，欲藉由文獻查證的方式來佐證此臨床上常用的護理診斷，除讓護理人員能更清楚的思考整個護理問題的原由，並且在照顧有這類問題的病人之同時，能夠更正確而有自信地提供患者更高品質且專業的護理。

Tissue perfusion altered-renal function is a nursing care problem often seen clinically. It is often mistaken as a minor problem or renal dysfunction, but as a matter of fact, tissue perfusion altered-renal function affects all bodily function and if early diagnosis is not made in time to allow for immediate monitoring and treatment, the result will be irreversible renal damage. The question of ”when should nursing diagnosis intervene?” has always puzzled clinic nurses and although a diagnosis has been adopted, it lacks clear definition. As a result, this paper attempts to explore the application of clinic diagnosis based on literature review to allow clinic nurses to understand and think of the cause of nursing care and to provide high quality and professional care to patients with accuracy and confidence.
