照護一位混合性結締組織疾病患者之護理經驗

Experience of Nursing Care on a Patient with Mixed Connective Tissue Disease

本文係運用Roy's適應理論模式照護一位混合性結締組織症女性患者。在護理過程中筆者藉由觀察及會談等方式進行整體評估，包括生理功能、自我概念、角色功能及相互依賴等不同層面。確立其護理問題如下：1.低效性呼吸型態 2.營養狀況改變：少於身體需要 3.無望感 4.潛在性危險性皮膚完整性受損等四項護理問題。針對個案的問題做整體性的護理計畫，並運用同理心建立良好護病關係，以護理人員的專業知識與技能協助個案，處理身心的健康問題，使個案可以積極正向的態度，面對疾病帶來的身心壓力與挑戰。

This article explores the nursing experience of caring for a female patient with mixed connective tissue disease by the application of the ”Roy's adaptation model” The author performed the broad-view evaluation, which included physiological function, self-concept, role function and interdependence, by clinical evaluation and interview during the nursing care process. Four nursing problems were established: Low-effectiveness of breathing pattern, Nutrition altered: less than body requirement, Potential skin integrity impairment and Hopelessness. A well nursing care plan was initiated according to patient's problems. With the help of specialized nursing knowledge and humanistic care, this patient could easily face her disease-related bio-psycho-social stress and challenge with more constructive and aggressive attitude.
