提昇氧療重症病人轉送過程完整性專案

An Integrated Patient Transfer Project for the Critical Care Patient Receiving Oxygen Therapy

氧療重症病人轉送過程中，易造成不同程度之併發症，礙於東部地形狹長交通不便，病人轉送路程費時，更增加風險，故本專案旨在提升氧療重症病人轉送過程的完整性。自2009年l月l日～2月15日，查檢「院內」及「院際」氧療重症病人轉送過程，發現完整性偏低原因為隨行設備不安全、缺乏操作標準規範及相關教育訓練。經由建制安全的隨行設備、制定氧療重症病人轉送操作標準、擬定監測計畫、舉辦在職教育等方案後將護理人員對院內轉送過程完整性由73.3%提升至100%；院際轉送過程完整性由74.3%提升至100%，改善效果顯著且達專案設定目標，未來仍持續修正改善方案，以提升護理人員轉送病人之安全性。

Critically ill patients with oxygen therapy develop different degrees of complications during the process of patient transfer. Especially in the eastern Taiwan, the geographical location causes traffic inconvenience, and long distance drive. They increase the risk for occurrence of the complications. The purpose of this project is to explore integration of the patient transfer process for critically-ill patients receiving oxygen therapy. We examined those patients who were transferred within the hospital or between hospitals during the period of January 1(superscript st) to February 15th, 2009. Some causes of low integrity include unsecured carrying equipment, lack of standardized operation, and shortage of related educational training. Establishment of safe carrying equipment, standardization of the operational transfer process for severely ill patients with oxygen therapy, patient transfer monitoring plan, and provision of in-service. The integrity of patient transfer process was improved from 73.3% to 100%. The transfer process between hospitals was raised from 74.3% to 100%. The improvement outcomes are significant. However, the future revision of the improvement project will promote nursing staffs safety during patient transfer.
