Critically ill patients with oxygen therapy develop different degrees of complications during the process of patient transfer. Especially in the eastern Taiwan, the geographical location causes traffic inconvenience, and long distance drive. They increase the risk for occurrence of the complications. The purpose of this project is to explore integration of the patient transfer process for critically-ill patients receiving oxygen therapy. We examined those patients who were transferred within the hospital or between hospitals during the period of January 1(superscript st) to February 15th, 2009. Some causes of low integrity include unsecured carrying equipment, lack of standardized operation, and shortage of related educational training. Establishment of safe carrying equipment, standardization of the operational transfer process for severely ill patients with oxygen therapy, patient transfer monitoring plan, and provision of in-service. The integrity of patient transfer process was improved from 73.3% to 100%. The transfer process between hospitals was raised from 74.3% to 100%. The improvement outcomes are significant. However, the future revision of the improvement project will promote nursing staffs safety during patient transfer. |
