初產婦與早產兒建立親子依附關係之護理經驗

A Nursing Experience of Building a Relationship between Primipara and Premature Infant

本文探討一位初產婦與早產兒建立親子依附關係之過程。個案因安胎失敗於妊娠33+3週自然分娩，筆者以羅氏適應模式收集評估個案資料，確立個案有照顧者角色緊張，及潛在危險性父母／嬰兒依附關係改變的健康問題。並依照個案需求，給予個別之護理措施，包括主動提供訊息；安排彈性探視時間；漸進引導照護相關知識與技巧；示範及引導個案觸摸案女，同時教導早產兒按摩技巧，並鼓勵個案哺餵母乳，最終成功降低個案緊張情緒，能勇敢撫抱案女並哺乳。

This article reports the process of building an attached relationship between the primipara and premature infant. The case unexpectedly and naturally labored after 33 weeks and three days of pregnancy and failure to stabilize the fetus. The author assessed the case based on the Roy Adaptation Model. Data were collected and analyzed. The identified health problems were role tension as a new caregiver and potential risk because of the change in the attached relationship between primipara and premature infant. Based on individual needs, customized nursing interventions included actively providing information, arranging flexible visiting hours, guiding care-relevant knowledge and skills gradually, demonstrating the case to touch the infant with message skills, and encouragement the case breast-feeding. Finally, the primipara could lower her tension and bravely touch and breastfeed her baby.
