子宮脫垂婦女決定接受子宮全切除術前之心路歷程

The Living Experiences ofWomen with Uterine Prolapse before Receiving Hysterectomy

本研究目的為瞭解子宮脫垂婦女從察覺身體出現異狀，至決定接受子宮全切除手術前之心路歷程。為質性研究，採立意取樣方式，選取七位診斷子宮脫垂的婦女並接受子宮全切除手術者，以深度訪談進行回溯性資料收集，二位研究對象訪談四次，四位研究對象訪談三次，一位研究對象訪談二次；每次時間約一小時，共得訪談實錄22份。並以內容分析法進行資料的分析與歸納。研究結果顯示子宮脫垂婦女決定接受子宮全切除術前之心路歷程共歸類為四個互動過程，包括：驚疑身體出現警訊；廣蒐資源尋求解決之道；兩難的抉擇擺盪；決定手術扭轉現況。本研究結果提供接受手術病患的決策經驗，期望有助於護理人員於臨床照護此類婦女之個別性護理需求參考。We design this research with an intention to understand the experiences of women with uterine prolapse from beginning to sense the physical differences till making decisions to undertake a hysterectomy through a qualitative research. Seven women who had been diagnosed to have uterine prolapse and were undergoing hysterectomy were recruited in this study by the purposive sampling. We used in-depth, retrospective interviews to collect the data. Two patients were interviewed for 4 times, four patients were interviewed for 3 times, and one patient was interviewed for 2 times. The interview duration was 1 hour. We obtained twenty-two materials in total. The data were analyzed by the content analysis and categorizing method. The results indicated that the women experienced four stages before making decision to undertake a hysterectomy: (1) realizing physical warning; (2) looking for information to solve the problems; (3) hesitating to undertake an operation or not; and (4) deciding to undertake an operation to improve the present situation. In this research we provide the experiences of the patients' decision-makings for surgery and hope that it is helpful for nursing practitioners' individualized care of such women.
