協助一位氣喘併發急性會厭炎患者脫離呼吸器之加護經驗

A Nursing Experience of Assisting an Asthma Patient with Acute Epiglottitis to Wean from the Ventilator in the Intensive Care Unit

本文為協助一位氣喘併發急性會厭炎患者脫離呼吸器之加護經驗，護理期間為2008年8月29日至9月2日共五天，筆者以高登十一項功能性健康型態評估架構，進行身、心、社會整合評估。個案於照護期間放置氣管內管使用呼吸器提升氧合能力，與個案共同制定呼吸訓練計畫成功脫離呼吸器，因個案對疾病認知不足提供正確知識，藉由關懷建立良好護病關係，鼓勵個案及家屬表達情緒及需求，以降低無力感，採正向態度面對疾病治療過程，教導疾病資訊之取得，增進對自身疾病的了解及日常生活預防及保健，期望藉此護理經驗提供日後照護此類病人之參考。

The case report described an ICU nursing of an asthma patient who suffered from acute epiglottitis in the ICU. From August 29 to September 2, 2008, the author applied Gordon’s Functional Health Patterns to conduct an integrated assessment of the patient’s physical, mental and social aspects. The patient was intubated and mechanically ventilated to increase oxygenation. The author established a breathing training plan in cooperation with the patient, allowing the patient to be able to wean from the ventilator successfully. The patient had insufficient knowledge about her disease. The author provided disease-related information to her and expressed concerns to establish a good nursing-patient’s relationship. The author encouraged the patient and her family to express their feelings and their demands, reduced her powerlessness and confront the disease treatment with a positive attitude. The author instructed the patient and her family to acquire information about the disease and self-care management of her disease in her daily life. We hope this nursing experience can serve as a reference for future nurses.
